You know how it is when you meet a new client, you want to chat and be friendly, but professional too. I am having real problems with getting the right mix, and I think I need a more defined form which will keep me on track.
Any suggestions please?

Realistically you need to gather all the info you need to do the work and bill it at that meeting...
It depends a bit on what you like to have - name, address, email, mobile, any PO numbers/billing instructions, any templates you should use and also any passwords/access codes you would need.
